Have you ever stopped and asked yourself “What if?” That’s how the first 10 minutes of Grey’s Anatomy season 8, episode 13 “If/Then” begins. As Meredith puts Zola to bed, she begins to wonder how her life (and the lives of those around her) would be different if her mom had never been sick or if she had a better father. Spoiler alert! Stop reading now if you don’t want to know the details of “If/Then.”

The “alternate” Meredith wakes up and bounds down the stairs dressed in pink and smiling from ear to ear. She tells her mother, “Good morning!” and tells her that she’ll ride to work with dad (Dr. Webber).
At the hospital, it becomes apparent that Ellis Grey is the Chief. She holds a meeting attended by Callie (who is a cardiac surgeon instead of ortho) and Owen (who are married with three kids), Derek and Addison (who have their first child on the way), a very timid and shy Dr. Bailey, Richard and Arizona (who doesn’t get along well with Callie).
As for Meredith, she is now engaged to Alex (who is just as bright and bubbly as she is), April is “her person,” and she can’t stand Cristina. Also, she makes it clear that she doesn’t like Derek, she calls him an “ass” after he brushes her off at the elevators.
Perhaps the biggest change is in Lexie. She’s brought into Seattle Grace’s ER on a stretcher. She’s a “coke head” who is covered in piercings and tattoos.
